COMPETITOR PRICING: $70 includes 3 events, $5 each additional event (Pre-Registration Pricing through April 30th, $65 includes 4 events)

SPECTATOR PRICING: $10 each (kids 5 & under are free)

Agreement, Release, and Waiver *

PARTICIPANT/ COMPETITOR WAIVER OF LIABILITY The Participant/Competitor agrees to comply with the rules and regulations of the May Madness Tournament, Michigan Sport Karate Circuit (web link to rules http://www.mskctour.com/Rules.aspx) and/or the listed events, collectively THE EVENTS. Participant/competitor acknowledges that participation and competition in THE EVENTS is physically strenuous and that participation in such competition can result in personal injury to a participant/competitor. The participant/competitor hereby waives any and all claims of damage or injury against Macomb Martial Arts, New Way Training Center, or any of their officers, agents, employees, sponsors, or any individual connected with the organization(s) or promotion of any of THE EVENTS, and participant/competitor expressly assumes all risks of whatsoever nature resulting from participation/competing therein. Additionally, I am fully aware of my personal medical conditions and hereby certify that I am mentally and physically fit to compete in the EVENTS. I agree to abide by the decisions of the May Madness Tournament medical staff at all times and that determinations regarding return to participation in competition by the medical staff are final. REFUND POLICY Participant/competitor acknowledges that by completing and purchasing registration or spectator tickets that the May Madness Tournament registration policy is no refunds and no transfers. All sales are final. PARITICPANT/COMPETITOR MEDIA WAIVER Participant/competitor acknowledges that by competing in or attending the EVENTS he or she hereby grants permission to use my image or likeness for future promotional purposes including photo, video, or audio recording or other likenesses or reproductions for media coverage, press releases, promotion or interviews utilized by those producing or directing this event at any time; and likewise waive any compensation of whatever kind for use of photo, video, or audio recording, or other likenesses or reproductions for media coverage, press releases, promotion or interviews utilized by those producing or directing this event at any time. Last, I hereby waive any compensation of whatever kind for use of pictures, video, or digital recording, or other likenesses or reproductions for media coverage, press releases or interviews utilized by those producing or directing this event at any time. AGREEMENT By authorizing this waiver, and prior to competing in the tournament, the competitor agrees and fully understands the rules, safety standards, age and weight requirements and will apply the policies outlined herein on any and all of EVENTS. The participant/competitor also agrees that responsibilities of sportsmanship, safety, and general common sense exceed further than the items and issues not detailed on this waiver and shall not be overlooked because it is not in written form. I understand and fully recognize the risk of physical injury or bodily harm in participating/competing in a martial arts tournament and with such knowledge and recognition; I hereby assume all risk of such injuries or death resulting from my participation/competing in the EVENTS. If I bring legal action in violation of the terms of this agreement, release, and waiver, I agree to pay the reasonable attorney fees and costs of the party against whom action is brought.
